If you are covered by a Health Management Organization (HMO), please contact your insurance to designate Dr. Tu as your primary provider before your first appointment to avoid cancellation or additional fees.

For your first appointment, please arrive early to complete intake forms. Also please bring your i.d. and a copy of your insurance card.

Depending on your insurance policy a copay may be due at the time of your visit. If you do not have insurance, we accept private payment for your care. Please call for more information.
